Every Secondary student in Sand Springs Public Schools MUST fill out a new Chromebook Agreement at the beginning of each school year agreeing to the upkeep and use of their SSPS issued Chromebook. This form is applicable to the 2023-2024 school year.

If you are filling out this form ahead of time and choose to participate in the SSPS Chromebook Insurance Program, you will still need to submit $30 at the Chromebook checkout events or to your student's school site.

SSPS Chromebook Insurance Program: *

I understand this program will reduce the cost for repair/theft that I may be subject to if the Chromebook on loan from the school is damaged or stolen. The costs of repair are described in detail on the Chromebook Insurance Agreement.

No, I decline participation in the SSPS CB Insurance Program. I understand that I will be financially responsible for the cost of all damage to the Chromebook or replacement cost in the event it is stolen while checked out to my child.
