Electronic registration for Chicago Cowboys 2020 Registration.  ALL participants will be required to submit an original birth certificate before the official Weigh in for season participation with the Cowboys.  Additional forms will be required for all new participants and distributed before the season starts.  Full registration is not complete until full payment, and ALL required forms are received. There are no refunds after August 1, 2020 and all COVID-19 guidelines must be followed for all. .

I hereby release and discharge the Chicago Cowboys, its board, coaches , appointed officials, league,volunteers from any and all claims or actions for losses, damages or personal injuries to myself of to my child which may occur or arise out of my, his or her participation in the activity named above. I herby give my consent to the Chicago Cowboys Youth Athletic Association for my child to participate in the above described activity. I realize that I will be required to volunteer for concession stand duty, participate in fundraising (as specified), or with  other areas.  I realize that I will be responsible for the prompt drop off and pick up of my chil(dren) from games/practices. Football and Cheer practice dates will be advised and will continue throughout the season or at the coaches' discretion.  The Board may enforce guidelines to ensure the safety of the program at any time. There are no refunds after 7/1/19. *
Parent/Guardian understands rental equipment is distributed for the Cowboys football season and must be returned at the end of the season or upon withdrawal of the player from the Cowboys organization.  if it isn't returned, the Cowboys reserve the right to pursue collection activites to retain its property. *
All parents are required to volunteer at least one hour at the Concession stand. If you decide to opt out of this duty, you will be required to pay a volunteer opt-out fee of $25. This is required with your registration. *
All parents/guardians are encouraged to serve on a committee.  Do you have any special area of expertise or just enjoy helping out?   *
There is a mandatory $100 fundraising fee. You can choose between two tracks to recoup your $100. Which track are you interested in? (Requires at least a deposit down towards your 2020 Registration in order to start). *
