For clients with BCBS, Cigna, or Anthem PPO insurance plans:

I understand that The Nest is partnered with The Lactation Network to provide insurance-covered visits to families who have select PPO plans through BCBS, Cigna, or Anthem. As the client, I am responsible for submitting my insurance information through The Lactation Network's website for authorization, and that it may take up to 24 hours to determine coverage. Without confirmation of coverage from The Lactation Network, I am fully responsible for the full cost of the visit.

For clients with an insurance policy that may provide reimbursement:

I understand that I can request a copy of the superbill from my appointment to submit to my insurance company for potential reimbursement. If reimbursement is not approved by my insurance company, I understand that I am still fully responsible for paying for the services rendered to me.

For all clients:
I understand that payment for office, virtual, and home visits are due in advance in order to secure my appointment. (Payment can be made in office prior to your appointment date or online, either on our website or via a link emailed to you.)

For all clients:

I understand that if I cancel my appointment with less than 24 hours notice, or if I fail to show up to my appointment, I will be charged a non-refundable $20 cancellation fee. I give permission for The Nest to store my credit card information and charge my card appropriately based on this policy. I understand that insurance will not reimburse this fee.
