Dear Parents,

The band directors are pleased that your child has shown interest in becoming a member of the Louisville Schools’ Instrumental Music Program. We are incredibly excited to start another large group of students this upcoming school year, and we believe that becoming a member of the band is a unique and important opportunity for your child!

This document is to inform you of the responsibilities of being a member of the Sixth Grade Band. At the end of this form, there will be a contract page for you and your child to fill out to help the band directors and middle school guidance department move forward with class scheduling.

Please submit your response of whether or not you want your student enrolled in band no later than Sunday, April 25th, 2021.
