Please complete all fields in order for us to have all the information we need to run your class.

Terms to understand-
Course = class (you need to fill out a new course proposal form for each course)
Session = same class, multiple dates (you fill out one form and give us all the dates of your course)
In-person = course that is live, together, in the same room class
Remotely = course that is over the computer, with the instructor on zoom/meet

Please NOTE: At the end of this form is a Note section. Anything you think we need to know that this form does not capture, please put in the note section.

Please be aware that this is a course PROPOSAL form, final details and offering the class are up to the discretion of the director. Any questions, additional information, or photos are sent to the director at lrobertson@yorkschools.org.

Thursday and Saturday class offerings need to be at your own studio location.(Examples of filling this out: If you want to teach on Tuesday evening, check Tuesday.
