2022/2023 CFM St. Joe's Registration
Hello Parish families! We are trying to simplify registration with this online form as our registration.
(Paper registration forms are available by contacting the CFM office)
Please fill out all parts of this form. Please provide Baptism, Reconciliation and First Communion dates (if applicable).

At our first CFM session, you will need to sign a paper copy of the release to participate in our program, copies will be provided at check-in. Registration forms (either online or paper form) are due August 26th for all families. Please contact the CFM office for financial assistance or to create a payment plan.
An additional $5 per child will be charged to any registrations received after the August 26th date.
 
We are looking for new and returning catechists! If you are planning on being a catechist, you still need to register your children by August 26th.

**NOTE - SCHEDULE CHANGE**
While K4-5th grade discipleship remains 1st and 3rd Sunday MORNINGS, (9-10:15am),  
6th-12th grade discipleship will now be held 1st and 3rd Sunday EVENINGS (6-8pm)
(There will NOT be an evening Mass)

First Eucharist Retreat (required of all who will receive their First Eucharist this year) $35
Confirmation Retreat (required of all who will receive their Confirmation this year) fee will be collected later
Fees waived for Catechist's children and St. Joseph's day-school students
Please check how many children you are registering in each category (For First Eucharist, please enumerate both "K4-5th" and "First Eucharist Preparation".  Likewise, for Confirmation, enumerate both "9th-12th" and "Confirmation"). *

Emergency Contact Information--In the event of an emergency, if you are unable to reach me, please contact the following person. Please include name, relationship to the person, and phone number. This needs to be someone other than the parents. *

I agree to read the family handbook prior to the first in person session on Sept. 19th. This will be updated and emailed to you by September 1st. *
I consent to the use by St. Joseph's Parish any videotape, photograph, slide, audiotape, or any other visual or audio reproduction in which I or my child/children may appear.  I understand that these materials may be used for promotion of the CFM programs.  Such promotional activities extend to recruitment, fundraising, advocacy, etc. I release the recording as part of any of the aforementioned or similar activities.   *
