Walk-a-thon 2024 Permission Slip

Please fill out one form per student by Wednesday, May 1st. A permission slip is REQUIRED even if your student is not walking after school. All students with a submitted permission slip can walk laps during Lunch Recess to count towards participation prizes.

In order for my child to participate after school, I understand that I must meet my child at Cherry Crest Elementary or have arranged adult supervision for my child if I cannot attend personally.

My child is not participating after school and will take normal transportation home.

All Kindergarten students need to be accompanied by a parent, guardian or chaperone to participate after school in Walk-a-thon.

MEDIA RELEASE: I hereby grant Cherry Crest PTSA all necessary licenses (on a royalty-free, irrevocable basis) to use likenesses, images (video, still or otherwise), and names of the undersigned and related children and dependents, obtained in connection with the 2024 Cherry Crest Walk-a-thon event, in Cherry Crest PTSA publicity and other materials. These materials can include all-school assemblies, annual Cherry Crest student yearbook, Cherry Crest family communications and the Cherry Crest PTSA website.
