AJA uses this form to confirm that people will be coming to our meetings.  The form is general and is used for meetings such as, Kaiwa Club dining meeting, Kaiwa Club social meeting and even AGM's. As this is a general form we will ask you to write which meeting you are attending so that AJA can confirm that it is correct. All questions are required to be answered before the form can be completed. Questions that must be answered will have a little star next to them.
