Each student and a parent/guardian are required to read the contract and complete this form separately to enroll in our 1:1 Mobile Device Initiative.

Completion of this electronic form acknowledges that you have read our technology policies and the 1:1 Mobile Device Program Initiative Agreement. (You do not need to print, sign, and return a physical form).

***A form submission is required from each student (grades 6 to 12) and a parent/guardian***

***If a parent/guardian has more than one student, please fill out the form for each student.***

***Please use the school issued STUDENT Google email from Holbrook School District, and for the parents/guardians use the same email provided to the school in Aspen.  If the emails do not match our records, you will not receive a mobile device for the student.***
