CHS 2024-2025 TI-84 Plus Calculator Loan Agreement for Needs-Based Situations
TI-84 calculators are used for all math classes as well as ACT and SAT. It is recommended that students own their own calculator, and label it with their name. If a student has a financial need and is unable to purchase a calculator, the MC has a limited number available for checkout.

**Please allow 24 hour turn around for each request to be filled. They will be delivered to your teacher, so please do not come to the MC to collect. Your email will be recorded when you submit this form.

TI-84 calculators are basic school supplies for HS students. They are used for all Math classes as well as the ACT and SAT. They can be purchased new or used from various vendors for a student to use throughout their educational career.  *
 If a student has a financial need and is unable to purchase a calculator, the MC has a limited number available for checkout.  *
I understand that any calculator that is damaged, lost, or stolen will cost me $154.90. Students are responsible for keeping up with the calculator and taking care of it while it is checked out to them.  *
Students are responsible for providing or replacing batteries for the calculator. CHS cannot supply batteries, so there may not be batteries in the one a student has checked out.  *
The calculator will be used for exams. Understand that your student must return the calculator to the Media Center immediately after the exam-do not leave with the Math teacher to return! *
It is the students responsibility to return the calculator to the Media Center and be sure it is cleared from their checkout record. If not, it will be a fee of $154.90 for the lost calculator.  *
