STUDENTS IN GRADES 3-12
 The school committee voted on August 26, 2020, to adjust our re-entry plan as follows:
* Students in grades PreK-one will return fully in-person, beginning September 16, 2020
* Students in grade two will return in a hybrid model, beginning September 16, 2020
* Students in grades 3-12 will return in a complete remote model, beginning September 16, 2020

Our goal continues to be the safe return of all students, fully in-person. Regular plan updates will be completed on a weekly basis. Additionally, it is essential for parents/guardians to understand that the situation is fluid, based on a number of factors. These include community and school metrics regarding the increase or waning of the pandemic. We also must act in conjunction with Governor Baker's and Commissioner Riley's mandates. Therefore, conditions will change accordingly and we will communicate the changes immediately to our stakeholders.

All families, regardless of the educational learning model selected, must complete this form. The form completion will help improve our ability to equalize class sizes and increase safety in our classrooms.

If you have more than one child, please complete this form for each child separately.

If you select an option and then later decide that it would be better for your child to learn in a different model, you must contact the appropriate building principal immediately to arrange a transition plan.
