By volunteering for Ballard FC and signing my name below, I am agreeing to abide by the above Code of Conduct. Furthermore, I confirm that I am in good health with no limitations on my ability to volunteer and hereby assume all risk of injury or harm as a result of any volunteer activities with Ballard FC. I agree to release, indemnify, defend, and forever discharge Ballard Football Club from all liability, claims, demands, damages, costs, expenses, and causes of action resulting from my participation.
