Student Teacher Request Form
If you would like to request to continue your field experience relationship with your teacher candidate and believe that the experience would be mutually rewarding, we encourage you, as a participating teacher, to consider requesting your candidate for a student teaching placement.  An extended time in the school will reduce the learning curve for all, will enable candidates to continue to build on the foundation you have provided and ensure that the evaluation of their efforts remains consistent.  Please note that per SUNY Geneseo School of Education policy, student teachers may not student teach in a school/district that they attended as a P-12 student.

This form MUST be completed by the prospective cooperating teacher (not the student teacher).

All individuals (teacher, candidate, and administrator) must agree to the assignment.  Please note that if you are employed in a district that requires central office approval, all requests will be directed to the central office contact prior to review by the SUNY Geneseo Office of Field Experiences.
