St. Madeleine Sophie Children's Ministry          Family Registration Form                                    2022-2023
This registration form is for both Kids' Church (3 yr old - kindergarten) and Whole Community Catechesis (WCC) elementary faith formation for 1st - 6th grade.

Kids' Church is for children aged 3 (by Sept 1st) through children in kindergarten offered during both Sunday Masses (8:30 am and 11:00 am). Lessons are developed to help understand basic concepts and rituals of our faith. Structured similar to the flow of Mass, the children hear sacred stories and are able to respond through art and play. Volunteers are needed in order to provide this program.

Our Whole Community Catechesis family faith formation program will be meeting in-person this year. Our sessions are structured to have the student attend with a mentor - generally a parent or other family member. The sessions will be on Wednesday evenings with meeting times below:

6:30 - 7:30 pm
  • 1st grade
  • Sacramental Prep (mostly 2nd gr)

6:30 - 8:00 pm
  • 3rd grade
  • 4th grade
  • 5th grade
  • 6th grade

7:30 - 8:00 pm - supplemental in Koinonia Room
  • 1st and Sac Prep children waiting for older siblings

Registration fees apply to Kids' Church and WCC:  $110/1 child, $160/2 children, $190/3 children
SMS School families: fee applies to Sacramental Preparation classes only

In consideration of this opportunity and fully recognizing that such an undertaking involves an element of risk, I assume all risks and hazards incidental to such participation and do hereby release, absolve, indemnify, and agree to hold harmless the Archdiocese of Seattle, Saint Madeleine Sophie Parish, their agents, employees and officers and the chaperones, leaders, organizers, sponsors and persons transporting my child to and from activities.  Neither the Archdiocese, the Parish, nor said persons shall be held financially responsible for any injury, illness, or death incurred as a direct result of these activities.  I, the undersigned, have read this release and understand all of its terms and execute it voluntarily and with full knowledge of its significance.  In the event of an emergency and I cannot be reached, I hereby authorize emergency medical treatment be administered. *
