Vacation Bible School Registration Form (2022)
Vacation Bible School (VBS) will take place from July 18th (Monday) to July 21st (Thursday) from 9AM-12PM daily. Each child or set of siblings (2 or more) can be "optionally" accompanied by at least one adult who must also be fully registered. VBS will host biblically themed and family/child friendly activities (while still respecting social distancing). These will take place inside the St. Paul School Cafeteria located at 140 Walnut St, Weirton, WV 26062. Please call the parish office at 304-723-2054 if you need assistance or more information.

*All Registration Forms must be completed & submitted for Pre-Registration (with payment) or RSVP NO LATER THAN Sunday, July 18th. Same day registration and payments will be accepted but pre-registration is highly encouraged as it helps us start our event on time.

**The fee is $46.00 per child for the full program, which covers food, craft supplies, a t-shirt, etc., or $10.00/day + $6.00 for the t-shirt. T-shirts are not optional as they have to be pre-ordered to have them in time for VBS week. Each child must have one. Please submit your payment through the free and super user-friendly Venmo app (our Venmo handle is "@stjosephtheworkerdre"), or you can mail a check to St. Joseph the Worker Parish or bring it in person at the parish office, located at 229 California Ave. in Weirton.

*** We will accept cash but we prefer & highly encourage cashless payments due to the pandemic.

**** Space is limited to 75 participants, so kindly pre-register or RSVP at your earliest convenience. Participants will be accepted on a first come, first served basis (We may hold a second VBS following an overwhelming response).

Vacation Bible School 2021 - St. Paul, St. Joseph the Worker, & Sacred Heart of Mary - Waiver (Release) of Liability and Hold Harmless (Indemnity) Agreement (Form): I do, hereby, via my electronic signature (First & Last Name) indicated below, waive all claims, release, indemnify, defend, safeguard, and hold harmless St. Paul, St. Joseph the Worker, Sacred Heart of Mary Catholic Church in Weirton, WV and the Diocese of Wheeling Charleston, its employees, and volunteers, from and against any claims, losses, damages, lawsuits, costs including, without limitation, attorney fees), and liabilities, in the case of my child/children – at Vacation Bible School 2021 volunteer(s) sustaining any injuries at a parish facility. *
Photo Release Form: I release and assign to St. Paul, St. Joseph the Worker, Sacred Heart of Mary Catholic Church, and the Diocese of Wheeling-Charleston all rights to the video, sound recordings, and/or photographs made by my child during Vacation Bible School hours and events. I authorize reproductions, sales, copyright, exhibition, broadcast and/or distribution of said video, sound recordings, and/or photographs without limitation for general religious and promotional purposes of the Diocese of Wheeling-Charleston. I release individually on behalf of my minor child, the Diocese of Wheeling-Charleston, its agents and employees from any and all claims, damages, liabilities, costs, and expenses which I now have or may hereafter have arising out of the making or use of such video, sound recordings, and/or photographs. I understand that I may withdraw this authorization in writing at any time. I further understand that refusing to grant this consent will in no way affect the scholastic or extracurricular services my child receives.   *
