Survey Instructions:

The United Way of North Central Oklahoma (UWNCO) is undertaking a Community Needs Assessment to help us understand the current community needs in Kay County. This Community Needs Assessment will allow us to gauge where community needs are being met, where gaps in services exist, and help determine where we can best invest our resources. This survey is anonymous, and your email will not be collected.

When answering these questions, please consider only those services that are located in Kay County. The survey should not take more than about 5 minutes to complete.
