Please complete the survey below to indicate your decision regarding the learning format for each of your children for the 2020-2021 school year. It is important to take into consideration the following information when making your selection. The State Board of Education has mandated students and staff wear masks/face shields at all times while in school buildings. Additional sanitation protocols are in place for the safety of everyone.  Tentatively, District 50 is planning to offer in-person instruction five days per week for a total of five hours per day. We kindly ask that your decision is made for the entire 1st quarter as we need to ensure appropriate planning and supports are in place for the needs of each child. We respect the decision each family makes as you know what is in the best interest of their child(ren).
