This document will provide the club with all essential information needed for the club to successfully register yourself or your child as a member of the club with Basketball England. It will also provide us with emergency contact information as well as giving you the option to provide permission for photo's/video's on occasion. All club documents are available on our website under the membership tab for your reference, as well as a downloadable hard copy of this form.
*PLEASE NOTE: In order for your child to be registered, you need to complete this form correctly. Full addresses, full names, correct information in the correct sections etc. The club will not accept registration forms if they are incorrectly completed. The club reserves the right to reject any applications or registrations throughout the season.
THIS FORM WILL NOT ASK YOU FOR PAYMENT. PAYMENT WILL BE REQUESTED VIA A SEPARATE INVOICING METHOD.
