**PLEASE NOTE - In order to be approved for adoption, all dogs/cats in your home must be spayed or neutered, Dogs must be current on Rabies and DistemperParvo (DHPP) vaccines and up to date on monthly Heartworm/Flea & Tick preventatives.  If there is a medical reason why your dog/cat cannot meet these criteria, please include that in this application. All of this information will be verified with your veterinarian.  By completing this application you understand and agree that others may be interested in the same dog as you and filling out this application and being approved doesn't guarantee you get that specific dog.
Also, understand that your application can be denied for ANY reason and Muttley Crue does not need to disclose why the denial. **

**ALL puppy adoptions (under 1 year of age) will require adopters to go through professional puppy classes within 6 months of adoption. This is to ensure you are raising the puppy to be the best dog they can be.  So please, before applying make sure you take this into consideration.**
