In connection to Lunchbox Theatre’s Code of Conduct LBT every effort is made to ensure that any complaint or concern is addressed appropriately and in a timely manner.
Filed reports are kept confidential and all feedback submitted through this form is received in confidence by General Manager Patti Neice and will be shared with Board Chair Duval Lang

All of the listed individuals are tasked with upholding the policies outlined in this Code of Conduct and will adhere to the Code’s outlined reporting and investigation systems. In the event of a departure from the staff of Board, this form will be updated with the new assigned representatives for the Organization.
Feedback without a request for follow up is welcome, and can assist the Company with determining patterns. If further action is requested by the person submitting the feedback or complaint, steps will be taken as appropriate to the situation on a case-by-case basis, determined in conversation between the individual issuing the concern and the Company Contact(s) who have received the information. This may include designated contacts interviewing the complainant, involved individual, and any other person who may provide productive context or information, a written record of the concern to be kept on file, or other steps. Information received will be held in the strictest confidence (when lawfully possible), and violations of this confidence will result in punishment or termination.
Upon submitting a complaint or concern, a reply acknowledging receipt will be issued in a timely manner of no longer than 2 business days. If requesting further action, the person submitting will be given an opportunity to choose from a selection of at least two different staff and board members to form a follow-up/investigation panel, with the additional option to request an outside third party. Please note, should you choose not to identify yourself during the reporting of the incident we will not be able to contact you, and may not be able to enforce the policy to the fullest extent.
Notwithstanding the above, in any extremely urgent and/or life-threatening situation in the workplace or theatre, particularly with regard to serious violence, the most important and immediate concern is the safety and well-being of the affected person. If there is imminent danger to a person’s safety, police, fire department or paramedics should be called.
