Legal Release and Policy Acceptance *

RELEASE AND WAIVER: I/we realize that participation in dance classes and activities could result in some possible personal injury. Despite precautions being taken by Dance tHerAPPY, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by Dance tHerAPPY. I/we will not hold Dance tHerAPPY, liable for any personal injury or any personal property damage/loss, which may occur on the premises before, during or after classes.

Class room policy: I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by Dance tHerAPPY.

Annual Registration Fee: A non-refundable registration fee is charged annually and covers all students in your family.

Media/Photography/Video: I/we give permission for my son/daughter’s photographs/videos to be used for publicity and advertising in the studio, on the Just for Kicks website, and in the community.

Billing Obligation: Sessions at Dance tHerAPPY are continuous. Tuition is to be paid monthly by the first of every month, per to services. A $20 late fee will be charged if tuition is not paid by the fifth of each month. No refunds will be given for any missed classes due to inclement weather, holidays, vacation, illness, etc.

Attendance Policy: No refunds will be given for any missed classes. Your tuition pays for a class spot, REGARDLESS OF ATTENDANCE. Make up classes are a privilege and we allow ONE make up per month. Make ups must be made within 2 weeks of the Missed class; otherwise the missed class is forfeited.

Dress code: Dancers are responsible for wearing appropriate practice attire to class.
