This form is for replacement hand-signed bookplates for Outrun the Wind, Red Labyrinth, and Lovestruck!

Please do not fill out this form if you did not purchase the December 2018, May 2019, or June 2019 Fae Crate boxes, or if you did not purchase one or all of these books during a Past Items Sale.
 
We understand that it is an extra step that you should not have to take by having to fill out this form, but unfortunately, this is the best option we have to ensure that everyone gets the correct replacements in a timely manner. Addresses or more may have changed in the past months.

We are currently estimating that the replacements will be shipped out within 4-6 weeks. We apologize that it will take so long to remedy this, but it will take time to get new bookplates to each author, have them signed, shipped back to us, and then turned around to ship out to each of you.
