Grapevine’s Children’s Ministry has put precautions in place to reduce the spread of COVID-19; however, the church cannot 100% guarantee that you or your child(ren), will not become exposed to or infected with COVID-19. Further, because of a number of individuals involved are children and the fact that many infected individuals may appear to be asymptomatic, attending a church-sponsored activity could increase your and your child(ren)’s risk of contracting COVID-19. By checking this box, you acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that your child(ren) and you may possibly be exposed to or infected by COVID-19. You also acknowledge and voluntarily assume the risk that by attending church activities, such exposure or infection may result in personal injury, illness, and disability. You understand that the risk of becoming exposed to or infected by COVID-19 at First Presbyterian Church may result from the actions, omissions, or negligence of yourself and others, including, but not limited to, church employees, volunteers, members, and participants and their families. On your behalf, and on behalf of your children, you hereby release, covenant not to sue, discharge, and hold harmless First Presbyterian Church of Grapevine, its employees, agents, and representatives, of and from the claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. You understand and agree that this release includes any claims based on the actions, omissions, or negligence of the First Presbyterian Church of Grapevine, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any activity or program. *
