During the course of the fashion show, you may submit a quote (text/copy), audio, pictures, and/or videos. By submitting those items, you grant Madison Reading Project, its volunteers, employees or representatives, permission to use photographs (including digital), video, text and audio of you in “promotional” materials and any of its programs or events including but not limited to the READ(y) to Wear event.

AGREEMENT

By checking the "I agree" box below, you acknowledge that you are 18 years or older and, agree and understand that you are participating in the Read(y) to Wear fashion show and fundraiser and will try to meet any deadlines set by Madison Reading Project. You also agree on behalf of your team that if the team, at any time is no longer able to participate and made a donation to the event, then the donation cannot be refunded but will be allocated toward the Madison Reading Project's General Program Fund.
