Welcome to Early Childhood Special Education (ECSE) in San Diego Unified School District (SDUSD)! We are here to support preschool-age children with developmental delays. This form will begin the process of referring your child for Special Education Assessment, which is the first step toward determining if your child is eligible to receive Special Education services in SDUSD. After completing the form, a member of our Intake Team will contact you to continue the referral process.

Please contact us at spedpreschoolreferrals@sandi.net to confirm that we received your request for referral. PLEASE DO NOT COMPLETE THIS FORM MORE THAN ONCE PER CHILD!

IF YOU ARE LOOKING FOR GENERAL EDUCATION ENROLLMENT and do NOT have any developmental concerns for your child that may require special education, please visit our Enrollment Options website for more information regarding Part-Day Preschool Programs for 3-year-olds or Universal Transitional Kindergarten (UTK) for 4-year-olds:

Please complete all sections [1-6]. All questions with an asterisk (*) are required in order to submit for processing. These questions are part of our typical enrollment process.
