This form is intended for complaints relating to inappropriate behaviour relating to athletes or others working in National Sporting Organisations (NSOs) with high performance carded athletes.

This form will be submitted to an independent lawyer for assessment.

Note: if you want to be contacted about your complaint and any resolution or outcome, you will need to provide your name and contact details.  If you do not provide contact details, the facilitator will have no way to contact you, and this may limit any actions which may be taken to address or resolve your complaint.
