You MUST complete your online application and submit documents BEFORE you complete this survey !  Fill out a different survey for each child.  The parent selections will be considered along with other variables including master scheduling.  Parent choices are not guaranteed or ranked, but are taken into consideration as we build balanced classes. NOTE:  5th grade students are departmentalized and will be assigned a homeroom teacher and rotate through each teacher during the school day, so no choice selections are needed.
