While we are excited about your decision, there are a few checks we must carry out. We'll require you to help us fill this form, but first, ensure that you meet the following requirements before you proceed.

1.  YOU MUST BE BORN AGAIN and able to explain your salvation testimony.
2.  You must be baptized through immersion and in right standing with the Lord.
3.  You must be musically inclined (i.e. you can sing, play one or more musical instruments or you can do both).
4.  You must have gone or be willing to go through at least one or more Discipleship Program(s).
5.  You should be a member of ECWA Wuse II.
6.  If you are not #5 above, you should be able to provide a "Letter of Introduction" to OUR Pastor from the Church you are coming from stating, if possible, the role(s) you performed in your former church.
7.  You should be older than 16 years
