Thank you for enrolling in our pilot program.  Please complete the below information.  A workbook and tools will be sent to you when we have reached our target enrollment number.  This FREE program is for adults 21 and over diagnosed with myasthenia gravis.  During this program we will explore six key areas of your life as part of a holistic lifestyle.    Completion of two surveys is required as part of the project.  One survey at the beginning and the second one at the end of the program.
