Thank you for your service as a mentor with Campuswide Pre-Medical Mentoring.  Both the program directors and pre-med students appreciate your expertise, your willingness to share and your time.

By offering to mentor with CPM, you are expected to connect with your mentee ONCE A MONTH.  

Please record any interactions you have with your mentor below.  CPM will send a monthly reminder to complete the form but feel free to complete it whenever you make contact.  Contact includes both conversations and attempts to connect with your mentee.
