Horizon Library - Reimbursement Form
This form is only to be used by Horizon libraries seeking reimbursement for items loaned to another Horizon Library.
Note that material which is eligible for the Automated Lost Reimbursement Process is not eligible for reimbursement via this form.
Items are only eligible for reimbursement for 1 year. Items marked missing by SELCO staff from a cleanup report are not eligible. SELCO staff will delete the item from Horizon when the reimbursement is processed.
