Mrs. Dunn's Lab Safety Rules & Contract 21-22                I, ______________________, a student in PAP Biology at Dayton High School, do hereby agree to follow all safety rules and regulations as set forth by the teacher, Mrs. Dunn.  I realize that compliance with these rules is necessary to assure the safe operation of the school laboratory and to provide a safe environment not only for myself, but also for my fellow students and for Mrs. Dunn as well.  I will, therefore, cooperate fully with the teacher and students to assure all of us the safest laboratory possible.                                                                   • I will always conduct myself in a responsible manner in the laboratory.                                                                        • I will study the laboratory activity before coming to the lab and will be prepared to ask                                       • I will dress accordingly for the lab environment, taking care to wear the proper clothing to class as well as wearing/utilizing all necessary personal protective equipment (PPE) for conducting the lab.                            • I will follow all written and verbal instructions carefully.  If I do not understand any part of a procedure, I will ask my instructor.                                       • I will work with my lab partners to ensure that all of us are equal participants in lab activities.                                • I will make sure my lab area is always clean and clear – any unnecessary items (book bags, electronic devices, unnecessary materials) will be removed from lab benches.                                                                                • I will not eat, drink, or chew gum in the lab                     • Upon entering a science lab, I will not touch any equipment, chemicals, or other materials until I am instructed to do so.                                                                   • I will handle chemicals carefully, check the label of every bottle or jar before removing the contents, and never return unused chemicals to their containers.               • I will clean my work area at the end of each lab.  I will not dispose of anything without first checking with my instructor.                                                                                    • I will report any accident (spill, breakage, etc.) or injury (cut, burn, etc.) to the teacher immediately, no matter how trivial it seems.  I will not panic.                      • I will respect and not waste the lab materials I am given to work with (e.g. gloves).                                            • I will never enter the storage room for any reason.                I understand that violation of safety rules will result in my immediate removal from the lab, a (0) for the lab grade, possible loss of laboratory privileges, and an automatic referral to the appropriate assistant principal.  Exceptions will not be made for anyone or any reason.                                                                     Student Signature:                                                             Date: *
