Thank you for registering for the 12HR Florida Chapter 491 Qualified Supervisor Training, CE Broker Course #20-609085, being held LIVE Via Zoom through Florida Art Therapy Services on Friday August 5 & Saturday August 6, 2022 9:00AM - 4:00PM with an hour for lunch. Please plan to log-in by 8:50AM the day of the training to resolve any technical challenges as we will begin promptly at 9AM.

You will be expected to have your camera on throughout the course and in an area free from distractions, as required by the Board. At the end of day 2, you will receive a survey and a quiz to complete.

Your certificate will be awarded and sent to the e-mail used to register for the course after the quiz has been submitted.

We will submit your attendance in CE Broker; however, it is recommended that you check the following week to ensure it was updated to your record.

Once this registration form AND payment have been received, we will send a confirmation e-mail to include the Zoom link information. Please save this email in a safe place so that you have the information to log-in. We will be sending an email with the Zoom link and PDF packet of materials within 24 hours of the course, as well.

Please note, we will NOT be able to reserve a seat without payment, and seating is limited to 40 registrants.

CANCELLATION POLICY: If you need to cancel your RSVP, please either call the office at 239-297-7099 or email reina@floridaarttherapyservices.com If the event is being offered at another time, your payment may be applied to the next time the event is offered. A cancellation with a request for a refund anytime prior to 7 days before the scheduled event will incur a $25.00 administrative fee. Cancellations requested within 7 days of the event or after the event will not be eligible for a refund. If you cancelled and opted to reschedule for the next event, and cancel again you will not be eligible for a refund.
