1. 此表格用作登記預留本節目門票。門票數量有限，每人限取 1 張。先到先得，額滿即止。每個電郵地址只限登記一次。This form serves for the registration of admission ticket of the captioned programme. Free tickets are distributed on first-come-first-served basis, with a maximum of 1 tickets for each patron. Single registration for each eligible email account.

2. 除個人資料(私隱)條例所訂明的豁免外，根據同一條例第18及22條及附表1第6項原則，你有權要求查閱及更改本表格內所載列的個人資料。Subject to exemptions under the Personal Data (Privacy) Ordinance, and in accordance with Section 18 and 22 and Principle 6 of Schedule 1 to the Ordinance, you have a right of access to and correction of the personal data provided in this form.
