Dear Students and Families,
This Google Form is the application for the Cunningham After School Program. This is NOT for the Millennium Program (that is a different application).
SHSAT Preparation classes are for Grade 8 ONLY! To apply for SHSAT, there will be a separate Google Form. Grade 8 students may still apply to join a club that does not meet on the SHSAT days you have requested on the separate form.
Parents/guardians MUST fill this out for their children in order to qualify for the program. After School Programs meet on specific days of the week. After School runs from 2:30pm-4:00pm on Tuesdays, Wednesdays, Thursdays, and/or Fridays. There are specific days in which clubs will be cancelled, so be sure to check the monthly Cunningham calendar (usually before holiday breaks). There are NO clubs open on Mondays.
Cunningham's After School Program will begin on Tuesday, September 26, 2023, for students who receive a letter.
There is NO YELLOW BUS service for grade 6 students who stay until 4pm. Every parent/guardian must make their own arrangements for students to commute home.
Applying does NOT mean that you have a seat. All applications are on a first come, first served basis. Every student who has a seat in club will be given a letter with details about the club and when it meets. If you do NOT receive a letter, you are NOT in the club. There will be a waitlist for every club and students will be given seats once they open up.
Any missing information on the application may result in not being offered a seat, so be sure to read and answer the prompts on the form carefully!
