Please complete the following form if you would like to meet with Dr. Scheibenhofer (principal), Mr. Spering (assistant principal) or Mrs. Brown (counselor).  We will be checking this form frequently throughout the day.  You will either get called down to the student office or will receive a pass/e-mail with a time to come for your scheduled appointment.

Reasons to complete this form:
1. Schedule change/question
2. Conflict with another student or students
3. General questions (academics, studying, PALMS questions, etc.)
4. Problem at home
5. Question about a club, activity or sport
6. Just want to talk!

Reasons not to complete this form and come directly to the student office:
1. You are at risk (self-harm, harm from someone else)
2. Someone else is at risk
3. You need to report a threat or something you overheard that could be dangerous
