I understand that a pre-participation sports physical is required for membership in Marching Band. I understand I need to have my family doctor do the physical before in-person rehearsals begin, and must fill out ALL 4 PAGES of the school's pre-participation physical form, as well as the HEALTH HISTORY UPDATE form and hand them in. I UNDERSTAND I MAY NOT BE ALLOWED TO PARTICIPATE IN IN-PERSON ACTIVITIES UNTIL MY PHYSICAL FORMS ARE COMPLETED
