I didn't understand what some of the questions were asking

I thought I knew how to do the problem, but actually didn't know how to do some parts of it

I thought I knew how to do the problem, but actually didn't add enough detail

I didn't listen in class: so I didn't know the answer at all

I missed the day this was taught, and forgot learn it/get help/take the notes after I returned

I never understood this in class, and didn't get extra help (FLEX time, Ms. Grant's office hours, email)

I learned this from somewhere other then Ms. Grant (friend, internet): turns out I learned it wrong.

I was over confident, and either rushed to finish or didn't check my work

I really just wanted to get it done and rushed.

I never practiced this type of problem (didn't do the homework/in-class work)

I thought I understood the problem, so I didn't take notes... Then I forgot how to do it.
