This form is for canceling your Membership with REAL RESULTS Fitness at The Plaza GYM
If you decide to return to REAL RESULTS Fitness at The Plaza GYM you will receive the most current pricing for your Membership.

Per our Cancellation Policy 30-days notice must be provided.
You understand that if you decide to cancel this agreement, You will give a 30 days notice prior to next billing date as stated in your membership agreement you signed. NO credits or prorating of paid dues will be returned to my account. There is a $20.00 cancellation fee if the cancellation is less than 30 days notice. Your account MUST be in good standing and active.
