This is the sign up form for Theta Chi's Rattler's Cup event on Saturday, April 9th. It is a 5v5 soccer style tournament with 20 minute games, two 10 minute halves. In the event of a tie, it will go to penalty kicks. The goal is to create a philanthropy event that involves the campus community.

In order to register, it is an individual fee of $20 per person. On this sign up sheet, you can sign up individually, as pairs, or up to teams of five players. Organizations that submit two teams will receive a sponsorship logo on the back of the shirt.

All players that sign up individually will be matched with other players that registered individually in order to create teams of five. At the day of the event, there will be brothers that fill in to ensure teams of five.
