2021 Evergreen Award - Vote Submission
The Forest of Reading is one of the few programs in Canada that puts the power in the hands of the readers by allowing them to choose the award winners.

Voting will be open September 1 - September 30. The 2021 Award Winner will be announced during Ontario Public Library Week in October.

How to Use This Form:

OPTION 1: Submit all your votes as a group.

OPTION 2: At-home readers can vote individually. Please put the number 1 in the total number of participants, the number 1 in the total number of voters, and enter 1 vote for the book you are choosing.
