By signing this consent form, I the undersigned participant, fully accept the following terms and conditions:

1. Participant takes part in the „EIT Health Infoday” organized by the EIT Health Partner: Institute of Transdisciplinary Discoveries (Medical School, University of Pécs) (hereinafter: RIS Hub) under the Regional Innovation Scheme activities implemented in partnership with EIT Health InnoStars (hereinafter InnoStars); / A résztvevő részt vesz az EIT Health Partner által szervezett "EIT Health Infoday": A Pécsi Tudományegyetem Transzdiszclipinárus Kutatások Intézete (a továbbiakban: RIS Hub) által az EIT Health InnoStars (a továbbiakban: InnoStars) partnerségben megvalósított Regionális Innovációs Program keretében.

2. No contract of employment, nor services contract, nor subcontract for work is concluded between the participant and RIS hub or EIT Health InnoStars e.V or EIT Health e.V for the activity mentioned under section 1; / A résztvevő és a RIS hub vagy az EIT Health InnoStars e.V. vagy az EIT Health e.V. között az 1. pontban említett tevékenységre vonatkozóan nem jön létre sem munkaszerződés, sem szolgáltatási szerződés, sem alvállalkozói szerződés;

3. The participant gives consent to EIT Health InnoStars, EIT Health, EIT and the RIS Hub to process his/her personal data for reporting purposes and in cases if it is necessary to comply with their audit/review obligations towards the relevant bodies. / A résztvevő beleegyezik, hogy az EIT Health InnoStars, az EIT Health, az EIT és a RIS Hub felhasználja személyes adatait statisztikai célokra, valamint olyan esetekben, amikor ez szükséges az érintett szervekkel szembeni ellenőrzési/felülvizsgálati kötelezettségeinek teljesítéséhez.
