I consent to SCRFC using photographs and/or video recordings including images of my child both internally and externally to promote the club.  These images could be used in print and digital media formats including print publications, websites, e-marketing, posters banners, advertising, film & social media.  I understand that images on websites can be viewed throughout the world and not just in the United Kingdom and that some overseas countries may not provide the same level of protection to the rights of individuals as EU/UK legislation provides.  I understand that some images or recordings may be kept permanently once they are published and be kept within the clubs archives. *

Medical Conditions / Injury History / Player Welfare Issues *
This includes any know allergies, physical or mental conditions.  Player welfare issues cover anything that may be relevant as we care for your child over an extended period of time whilst they are interacting as part of a large group.  This information is confidential to the coaching group.  Please mark as N/A if no Medical Conditions or history of major injury is present.  
As with all sport there is an risk of injury in participation.  For this we must ask that consent is given for basic first aid to be administered and emergency treatment if required.  Please note that we will not be able to accept the players attendance at the camp without this consent. *
