FFWNY RLT Operating Principles & Stipend Agreement
Thank you for becoming a Regional Leadership Team member for Food Future WNY. We appreciate your passion and interest in strengthening the region's food system. 

Attached to this form are the Operating Principles Agreement and the Stipend declaration form. 

By acknowledging and agreeing to the Operating Principles Agreement, you are indicating your "official" participation on the Regional Leadership Team. 

Please read and acknowledge the Stipend declaration form, as well as noting whether you accept or decline the stipend.

If accepting a stipend, we ask that you also fill out a W-9 form, which is linked below. Please fill out the form, sign it, and return by email to Kristen Tim at kristen@sustainagriculturenys.org.
