Physical Activity Readiness Questionnaire (PAR-Q)
Regular physical activity is fun and healthy, and the completion of a PAR-Q is a sensible first step to take if you are planning to increase the amount of physical activity your child does in their life. Being more active is safe for most people. However, some people should check with their doctor before they start becoming much more physically active.

Before your child takes part in Emily’s Pole Fitness' classes which consist of various poses and tricks using one or all of the following pieces of equipment: aerial silks, aerial hoop, static trapeze or poi; and consist of aerobic exercise, dance, toning and muscular strength building exercises, please read the questions below carefully and answer honestly by selecting YES or NO:

If you answered NO honestly to all PAR-Q questions, you can be reasonably sure that your child can start becoming much more physically active - they will begin slowly and build up gradually.
