In the name of Allah (swt) and the Ahl-ul-Bayt (as), it is our hope that everyone will answer all the questions in this survey honestly, and keep the education, safety, and well being of our whole community in your mind and heart.

The purpose of this survey is to collect information and feedback on whether IHS members have been vaccinated, and feel safe to return to in-person schooling, given the current COVID-19 pandemic guidelines.

The final decision for which format (in-person or online) IHS will be conducted in for the upcoming academic year will take into account several factors, of which one will be the results of this survey. Several other factors, including how local school districts are conducting the education process will be considered for the final decision. The final decision will be made in the best interest of everyone's education, safety, and well being, insh'Allah.

Guidelines and protocols include: wearing a mask at all times for everyone 2 years of age and older, social distancing, no eating or drinking, no physical contact with anyone, and other guidelines and protocols put in place as needed.
