This form is to be filled out only by the parents of students currently enrolled in TES in the 2021-2022 school year who have siblings who would like to attend in 2022-23.  This form is being used for planning purposes.  (Re-enrolling currently enrolled children is done on a different form, "The Intent to Return" form.)

You will have the ability to enter multiple siblings, if applicable.  This form is due by February 4th.  Only siblings entered on this form will be included in the sibling lottery for seats for next year.  While siblings receive priority placement, placement is based on seats available.  Therefore, not all siblings are guaranteed a seat.  Siblings must be listed on this form by February 4th, 2022 to receive preference.

Sibling defined according to NCGS 115C-218.45(f): the term “siblings” includes any of the following who reside in the same household: half siblings, step siblings, and children residing in a family foster home."
