Learning Format Request Form
For the fall of 2020, i3 Academy is taking a multifaceted approach that offers three options to our families. The options are displayed in the diagram below. Each family will choose a learning format that best meets their needs. By July 24th, families must submit this form. On August 7th, parents will receive an email confirming the learning format. We will try to accommodate all requests. If we do not receive a completed Learning Format Request Form, we will assume that your child will attend school in person. Approximately every four weeks, families will have the option to change their child's learning format. More information will be shared about the Change Format Form in the weeks to come.
Every stakeholder must do their part to keep students and faculty safe and healthy. We are asking families to agree to the following:
+ I will not send my child to school if he/she has symptoms of COVID-19.
+ I will communicate with the school/my child's teacher if my child must miss school or shows symptoms of COVID-19.
+ I will check in with my child’s teacher on the days he/she is not on campus.(hybrid/remote only)
+ I will review school and CDC social distancing guidelines with my child.
+ I will encourage my child to wash hands or use hand sanitizer regularly.
+ I will allow my child’s temperature to be taken and recorded at school.
