Artist Entry Form
Thank you for your interest in agreeing to supply artwork to the 2023 Aurtism Aurt Show. Living La Vida Autismo has partnered with Lucky Duck and Coloring Life My Way to help support autism awareness in the community.  The Lucky Duck is hosting a painting workshop for children and young adults on March 11 at 1:00 p.m. that will allow talented artists who are diagnosed with autism to create some amazing works of art. These works will be featured in the "Aurtism Aurt Show” Silent Auction on Saturday, April 1 at 5 p.m. at the Lucky Duck, 100 S. Main St. #114 Duncanville TX 75116. Proceeds will be donated to Coloring Life My Way nonprofit organization.  All are invited to attend this special event.

Important Information
  • If your child is unable to participate in the art workshop, you have the choice to either create your own artwork at home with your own supplies or pick up an art kit between February 28 - March 11 at Luck Duck.  All artwork must be 3 by 2 size. 
  • All artwork can be submitted on March 24, 25 or 27th at Lucky Duck.
  • If you choose to create artwork on paper, please put it in a frame. 
  • If you choose an art kit, please adhere to the following: Paint and a canvas will be provided.  Please ensure that you cover the table and the child's clothing. 
  • All displayed artwork will have the child's first name, last name initial and the city they are from

I give permission for my child to be photographed and/or videotaped and later published in print or electronic media.   I acknowledge and agree that photographs and videos may be edited and used in whole or in part as desired for these purposes, and may be produced, duplicated, distributed and used for informational, promotional, or other public purposes.  I understand that photographs and videos are not my property and I will not be compensated for them.  I understand and authorize the use in writing or otherwise the name or identity of said artist.
