The Greater Baltimore Urban League appreciates your and your child's interest in this program. This application is intended as a means of informing and gaining the consent of the parent/guardian to allow their child to participate in the Saturday Leadership Program.

I give my informed consent and permission for my child to participate in the Saturday Leadership Program and its related activities.
1. My child will follow all Saturday Leadership Program guidelines.
2. I authorize the Saturday Leadership Program to use any photographic image or name of my child taken while participating in the program. These images may be used on the Greater Baltimore Urban League website, newsletters, or other promotional material.
3. I authorize the Saturday Leadership Program to use any student products created or interviews conducted during Saturday Leadership sessions for the Greater Baltimore Urban League website, newsletters, or other promotional material.
4. I authorize the Saturday Leadership Program to request school grades and attendance records for my child to analyze progress and better tailor the program to the participants it serves.
5. I authorize the Saturday Leadership Program to collect feedback surveys from my child or session facilitators to analyze progress and better tailor the program to the participants it serves.
6. The grade, attendance, and feedback survey records may be summarized for the Saturday Leadership Program as a whole and released in Greater Baltimore Urban League annual reports, Gala publications, or grant reports. My child’s name or individual record will be kept confidential and never released.
7. To better help us help your child, we may request your child’s IEP or 504 Plan. Providing this is completely voluntary and will not affect your child’s participation in the Saturday Leadership Program.
8. I can request to review questions included in the feedback surveys.
